Please call (02) 9419 5311 to make an appointment with Dr Ian Farey, Monday - Friday between 8-4pm.

 

Before your appointment can be confirmed, you will require a referral from either your General Practitioner or another Specialist Physician.

 

Please arrive at least 10 minutes before your appointment time to complete the Patient Registration Form.  Alternatively, you can download and complete this form and bring it with you on the day.

Please remember to bring the following to your appointment with Dr Ian Farey:

• Completed New Patient Registration Form

• Referral letter from your General Practitioner (valid fore 12 months) or another Specialist (valid for 3 months).  If you do not have a referral letter you will not be able to claim a rebate from Medicare

• Information relating to previous surgeries

• X-rays, MRIs or CT scans

• A complete list of medications you are currently taking

 

Your Account:

All consultations with Dr Ian Farey should be paid in full on the day of service.
Should you have questions about the consultation fees, you can discuss these in confidence with Dr Ian Farey's Practice Manager on (02) 9419 5311.
 

Cancellations:

Please notify the Practice Manager on (02) 9419 5311 at least 24 hours prior so that your appointment time can be allocated to another patient who is seeking treatment.
